DES 505 Research Project Draft Survey

 Below is the current draft of the survey that I plan to be sending out to collect data from Bay Area communities. I am hoping that with the data I receive, it will assist me in addressing important elements and types of information that is essential to communities that will benefit from WiseHealthSf's community program that is Healthy Street.
